The form communicates detailed instructions about which aspects of the car should be worked on including check boxes for wash/detail, clean interior, undercoat, rustproof, state inspection, oil change & filter, radio, tires, brakes, wheels, moldings, tune engine, and lines for a detailed description of the service required. The top of the form includes spaces to complete information about the vehicle, salesperson, and customer. The bottom of the form includes a space to indicate the date the customer was promised for delivery.
